What is Support at Home?

Support at Home refers to government funded services designed to help older Australians remain living safely and independently in their own homes for as long as possible. It brings together funded supports that were previously delivered through different programs into a more coordinated approach, with a stronger focus on individual choice, flexibility and continuity of care.

Under this model, eligible individuals receive approved funding to support daily living, personal care and wellbeing needs. Support at home services are intended to adapt as circumstances change, working within the framework of the Strengthened Aged Care Quality Standards to promote quality, safety, dignity and person centred care.

Who is it for?

Support at home services are suited to older Australians who have been approved for a Home Care Package and require assistance to use their funding effectively, as well as those waiting for services to commence or experiencing delays in accessing funded supports. It can also support people whose needs have increased over time and are no longer fully met through existing arrangements.

This type of support may be helpful for individuals who require help with daily living activities, personal care, transport, social support or short term assistance following illness, injury or hospital discharge. It is also appropriate for people who prefer greater flexibility or continuity in who provides their care, particularly where consistency and familiarity are important.

Support at home services can also assist family members and carers who need additional help to sustain their caring role. By working alongside funded care arrangements, this support helps ensure care remains responsive, person centred and aligned with changing needs and priorities.
